BRIEF INTRODUCTORY PHONE CONSULTATION
Free of charge/15 min
This brief intro phone consultation is an opportunity for you to ask questions about my practice. This phone call is also an opportunity for me to screen potential clients so that I can be confident I am the right fit for you. If we both agree to move forward and work together following this phone call, we will schedule a time for your initial intake appointment. I will then send you via the secure Client Portal the New Client Paperwork necessary to get started.
INDIVIDUAL PSYCHOTHERAPY
$325/45 min
I offer both short-term (8-12 sessions) and long-term psychotherapy. Prorated 60-minute therapy sessions are available upon request. To effectively achieve the most successful treatment outcome, weekly individual appointments are recommended to start; bi-weekly appointments may typically be expected as one continues to progress and grow. Depending on different presenting concerns and levels of care clients may need, the length and duration of treatment vary from one individual to another.
CAREER/LIFE COACHING
$325/45 min
Offer to those who are interested in goal-oriented, purpose-driven coaching sessions aiming for career advancement and personal growth. Topics may include: career uncertainty, job/life dissatisfaction, fear of the unknown/unexpected turns, goal setting, action plan, make difficult decisions, move career toward a clear direction, reach one’s full potential, etc. Prorated 60-minute coaching sessions can also be arranged upon request.
COMPREHENSIVE CONSULTATION
$325/45 min
Offer to individuals who may not need ongoing psychotherapy but will benefit from one or a few comprehensive/psychoeducational sessions to consult about a specific issue. Prorated 60-minute comprehensive consultation sessions can be arranged based on my availability.
CLINICAL SUPERVISION & CONSULTATION

I offer clinical supervision and consultation to clinicians in training, licensed clinicians and those in the helping, teaching and health professions. Please reach out for more details if interested.
Note: Dr. JM currently uses a confidential HIPAA-compliant telehealth platform for all sessions. Once you schedule an official appointment, I will send you via the secure Client Portal the New Client Paperwork necessary to get started. A video appointment link will also be sent to you before your scheduled time.
Clients with previous therapy experiences often report having made progress and moving toward intended results since the initial stage of our work together. If you are serious about making meaningful change and committed to set goals and take actions for your growth, we might be a good match.
INSURANCE
My practice focuses on providing genuine care and individualized treatment. I do not participate in health insurance networks and do not bill insurance directly. However, if your insurance has a PPO plan, I will be glad to to provide superbills for you to seek your Out-of-Network (OON) reimbursement.

I am considered an Out-of-Network (OON) provider. Being an OON provider allows me to use my time directly to help clients and to maintain greater client privacy. Keep in mind that for you to submit your OON claim for your psychotherapy sessions, I must give you a mental health diagnosis that will stay on your permanent health record. In some cases, your insurance company may also ask for more details of your treatment. For those who want to ensure complete privacy and/or do not have diagnosable mental health disorders, they often choose not to file OON claims. Coaching/comprehensive consultation sessions are not covered by OON benefits.
If your insurance has a PPO plan and you intent to file OON claims, please contact your insurance company directly about your OON benefits. Here are some key questions you may want to consult your insurance company:
1. Do I have Out-of-Network mental health benefits?
2. Are telehealth sessions covered?
3. Is individual psychotherapy (CPT codes 90834, 90837) covered?
4. Do any CPT codes require pre-approval?
5. What is my deductible amount and has it been met?
6. What is the reimbursement rate for my OON mental health services?
7. Is there a maximum allowed amount per session?
8. How many therapy sessions per calendar year does my plan cover?
9. How do I submit an OON claim?

Good Faith Estimate (Per the No Surprises Act of Jan. 2022)
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
